LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. The ideal candidate will have strong communication skills, knowledge of HIV-related issues, and a passion for advocacy.
Responsibilities:
- Support individuals living with HIV and connect them to care.
- Conduct outreach and educational initiatives.
- Build relationships with community organizations.
- Advocate for patients and address barriers to care.
- Provide HIV-related education and resources.
- Collaborate with healthcare teams to support patients.
- Maintain records and complete assigned tasks.
Qualifications:
- Experience in community outreach or healthcare.
- Knowledge of HIV prevention and support services.
- Strong interpersonal and organizational skills.
- Bilingual in Haitian Creole (preferred).
