Description of the role:
University Home Care is seeking a Licensed Master Social Worker (LMSW) for our location in Livonia, MI. The LMSW will be responsible for providing comprehensive social work services to patients in their homes.
Responsibilities:
- Conduct initial patient assessments and develop personalized care plans
- Provide counseling and support to patients and their families
- Coordinate with healthcare providers to ensure holistic care for patients
- Advocate on behalf of patients to access necessary resources
- Maintain accurate and up-to-date patient records
Requirements:
- Valid LMSW license in the state of Michigan
- Prior experience in home care or healthcare setting preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
