Job Summary:
As a Hospice Nurse, you will coordinate, facilitate, and perform interdisciplinary care plans through thorough assessment and evaluation of patient conditions. You will travel to patient homes, Community-Based Residential Facilities (CBRF), assisted living locations, and hospitals to meet with patients and their families, ensuring their care needs are met in accordance with policies and regulations.
Key Responsibilities:
- Assess and coordinate patient discharge planning needs with the healthcare team.
- Perform point-of-care testing as per policies and procedures.
- Collaborate with physicians on care plans and provide regular updates on patient status.
- Communicate with multidisciplinary team members and outside agencies to achieve optimal patient outcomes.
- Complete admission paperwork and comprehensive medical assessments in a timely manner.
- Provide skilled nursing care, including vital signs, wound care, IV care, and more.
- Document visit notes and ensure all records are accurate and complete.
- Develop comprehensive care plans in collaboration with patients and families.
- Maintain a constant state of alertness and safety.
- Perform other duties as assigned.
Qualifications:
- Education: Graduate of an accredited school of nursing or equivalent.
- Experience: No prior experience required.
- Physical Requirements: Ability to communicate effectively, stand, walk, lift, and perform various physical tasks.
- Licenses/Certifications:
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Professional Nurse (RN) - State-specific licensing required.
- Valid Driver's License and clean driving record.
Working Conditions:
This position is a full-time role with a day shift schedule, requiring travel to various locations within the designated area.
